Annual Financial Disclosure & Affiliation Statement for Elected Officials, Appointed Officials, Directors, and BCC Staff

IMPORTANT: This form is ONLY for elected officials, appointed officials, directors, and City staff serving as Board, Committee, or Commission (BCC) liaisons for the City of Evanston.

Section 1-10-3 of the City Code stipulates that each elected official, appointed official, director, and employee who staffs a Board, Committee, or Commission (BCC) in the City of Evanston is required to complete and submit an Annual Financial Disclosure and Affiliation Statement through the Evanston City Clerk’s Office.

NOTE: This requirement is separate and independent from any annual filing requirements with the Cook County Clerk’s Office, if applicable.
